  • Complete the application for a duplicate title.
  • If your title is damaged or destroyed make sure to attach it to the application.
  • Make sure to include the make and identification number as well as the vehicle year and the current odometer reading.
  • If you have a lienholder on your title you must include a completed and notarized lien release form with your application. You can either download the form or get it at your local county treasurer's motor vehicle office.
