  • If the title doesn't show the selling price and date of sale then provide a completed Bill of Sale signed by both current and new owner and get it notarized.
  • An Odometer Disclosure Statement if the title does not include this information and the vehicle is less than ten years old.
  • Proof of Vehicle Identification Number Inspection completed by an authorized person.
  • Power of Attorney if the application is being made through mail.
  • An optional Title Application, which may be submitted by Out-of-state dealers, lienholders, and leasing companies.
